Camden Foundation was integral in getting a bill passed in the state of Illinois proposing the issuance of specialty license plates benefiting St. Jude Children’s Research Hospital. Now we need your help! Before production of these plates can begin, 1,500 “requests” for the St. Jude plates need to be submitted to the Illinois Secretary of State. You can find the request form enclosed in this newsletter or at the following website: http://www.cyberdriveillinois.com/publications/pdf_publications/vsd702.pdf.
Each request needs to be completed and returned to the Secretary of State’s office or returned to Camden Foundation for mass submission. The name of the specialty plate being requested should read “St. Jude Children’s Research Hospital.” A $25.00 specialty plate donation payable to Secretary of State should accompany each request. All donations will be given to St. Jude.
Completion and submission of this form does not mean that you are required or obligated to purchase the plates if they are produced. When the plates are available for purchase, $25.00 from each plate purchase and renewal will be donated to St. Jude. This gift keeps on giving!
